Primary Clinic Senior Care is a specialized virtual primary care practice designed exclusively for Medicare beneficiaries. We focus on providing proactive, comprehensive healthcare to older adults, helping them manage complex chronic conditions, avoid unnecessary hospitalizations, and remain safely and independently at home.

Yes. We are a fully licensed medical practice staffed by board-certified providers. While our care is delivered virtually to maximize your convenience and safety, we provide the exact same high-quality primary care you would receive in a traditional brick-and-mortar doctor's office.

Older adults often face unique, complex healthcare challenges that require more time, attention, and coordination than traditional clinics can provide. By focusing exclusively on Medicare beneficiaries, we have built a model tailored to the needs of aging—emphasizing chronic disease management, fall prevention, medication optimization, and deep collaboration with home health agencies.

2. Traditional Medicare

Yes, we proudly accept Traditional Medicare (Part B) and most Medicare Advantage plans. Our goal is to maximize the benefits you have already earned to provide you with exceptional, continuous care.

Most of our core services, including virtual primary care visits, Annual Wellness Visits, Chronic Care Management, and Advanced Primary Care Management (APCM), are covered benefits under Medicare. Standard deductibles and copays may apply depending on your specific plan. Our team will verify your benefits before you begin.

While a supplemental plan (Medigap) can help cover standard Medicare copays and deductibles, it is not required to become a patient. We work with patients who have Traditional Medicare alone, as well as those with supplemental coverage or Medicare Advantage plans.

3. Telehealth Visits

A virtual primary care visit (or telehealth visit) allows you to see and speak with your provider using a smartphone, tablet, or computer from the comfort of your own home. It functions just like an in-person visit—we discuss your symptoms, review your medications, order tests if necessary, and develop a treatment plan.

Absolutely. We understand that technology can sometimes be frustrating. Our platform is designed to be extremely senior-friendly. We send a simple, secure link via text message or email—you just click it to join the visit. If you ever have trouble, our care coordination team will walk you through the process step-by-step over the phone.

Yes! We highly encourage family involvement. With your permission, family members or caregivers can join your virtual visit from their own devices, regardless of where they live. This ensures everyone is on the same page regarding your care plan.

4. Becoming a Patient

Enrolling is simple. You can click the 'Become a Patient' button on our website to submit your basic contact information, or you can call us directly at 888.991.4911. Our care coordination team will reach out to schedule an initial consultation and collect your medical history.

Yes. We act as your primary care provider, meaning we coordinate your overall care. You are absolutely free to continue seeing your existing cardiologists, pulmonologists, or any other specialists. In fact, we will actively communicate with them to ensure your care is aligned.

We prioritize immediate access. We typically schedule initial onboarding appointments within a few days of enrollment, ensuring you don't have to wait weeks or months to establish care.

5. Appointments

You can schedule an appointment by calling our care coordination team at 888.991.4911 or by sending a secure message through your patient portal. We offer prompt scheduling, often with same-day or next-day availability for urgent needs.

Unlike traditional clinics that rush you out in 10 minutes, our appointments are unhurried. We take the time necessary to address all your questions, fully review your medical history, and explain your treatment plan thoroughly.

If you need to reschedule, simply call our office at least 24 hours in advance. We understand that life happens, and we will gladly find a new time that works for you.

6. Primary Care Services

We treat a wide range of acute and chronic conditions. This includes managing high blood pressure, diabetes, heart failure, COPD, arthritis, and depression, as well as treating acute issues like respiratory infections, urinary tract infections, and minor injuries.

Yes. Our providers are fully licensed to prescribe medications when clinically appropriate. We send prescriptions electronically directly to your preferred local or mail-order pharmacy.

Yes. We understand that physical and mental health are deeply connected. We provide integrated support for depression, anxiety, grief, and caregiver stress as a core part of your primary care plan.

7. Medication Management

Yes. Comprehensive medication reconciliation is a cornerstone of our care. Seniors often suffer from 'polypharmacy' (taking too many medications). We carefully review every pill you take to reduce dangerous interactions, eliminate unnecessary drugs, and simplify your daily routine.

You can request a refill by calling our office or sending a message through the patient portal. Please try to request refills a few days before you run out to ensure there is no gap in your treatment.

8. Laboratory Testing

When your provider orders blood work or other laboratory tests, we coordinate with local laboratory facilities (such as Quest Diagnostics or LabCorp) near your home. You simply visit the local draw station at your convenience, and the results are sent securely back to us.

In many cases, yes. If you are homebound or receiving home health services, we can often coordinate with mobile phlebotomy services or your home health nurses to have labs drawn right in your living room.

9. Imaging

If imaging is necessary, our providers will electronically send the order to an imaging center or hospital convenient to you. For some homebound patients, we can also coordinate mobile X-ray or ultrasound services to be performed at your residence.

10. Referrals

Depending on your specific Medicare Advantage plan (such as an HMO), you may need a formal referral. If so, we will process the referral for you. For Traditional Medicare, a formal referral is not usually required, but we will always help you find the right specialist and send them your medical records.

Absolutely. Fragmented care is dangerous. We proactively share your care plan and recent visit notes with your specialists to ensure your entire healthcare team is working toward the same goals.

11. Home Health Coordination

We consider home health nurses and therapists to be critical partners. We serve as the responsive physician team they need. We review and sign orders promptly, answer clinical questions from the field, and adjust care plans in real-time to keep you healthy at home.

Yes. You have the right to choose any Medicare-certified home health agency. If you do not have one and qualify for services, we can help coordinate a referral to a highly-rated agency in your community.

12. Advanced Primary Care Management (APCM)

APCM is a Medicare program designed for patients who need ongoing support between regular doctor visits. It allows our team to dedicate time each month to coordinating your care, managing your chronic conditions, reviewing your medications, and communicating with your specialists and family.

Yes, APCM is a recognized Medicare benefit for eligible beneficiaries. Depending on your specific plan, standard coinsurance or deductibles may apply. We will discuss the program with you and verify your benefits before enrolling you in APCM services.

13. Remote Patient Monitoring (RPM)

RPM involves using secure, cellular-connected medical devices (like a blood pressure cuff or weight scale) that you use at home. These devices automatically transmit your vital signs directly to our clinical team on a daily basis.

It allows us to detect subtle changes in your health before they become emergencies. For example, if we see a sudden spike in your blood pressure or a rapid increase in weight (which could indicate heart failure fluid retention), we can intervene immediately, often preventing a trip to the emergency room.

14. Annual Wellness Visits

An Annual Wellness Visit is a yearly appointment dedicated entirely to preventive care. It is not a standard physical exam; instead, we use this time to update your health risk assessment, review your medical history, screen for cognitive impairment, and develop a personalized, 5-year prevention plan.

For the vast majority of patients with Traditional Medicare, the Annual Wellness Visit is covered at 100% with no copay or deductible, as long as it has been at least 12 months since your last one.

15. Care Coordination

If you are admitted to the hospital, we want to know right away. While we do not round in the hospital, we will communicate with your hospital doctors to provide your medical history. More importantly, we provide rapid Transitional Care Management after you are discharged to ensure your medications are correct and you are stabilizing at home.

Yes. If you need Durable Medical Equipment (DME) such as a walker, oxygen, or a hospital bed, our care coordinators will write the necessary orders and work with local suppliers to have it delivered to your home.

16. Communication

You can call our clinic at 888.991.4911 during normal business hours, or you can send a secure message anytime through your patient portal. We pride ourselves on returning calls and messages promptly.

Yes, provided we have your documented consent (via a HIPAA Release of Information form). We frequently update designated family members and caregivers, helping relieve their stress by keeping them informed about your care plan.

17. Billing

We bill your Medicare or Medicare Advantage plan directly for the clinical services we provide. You will only receive a bill from us for any applicable deductibles, copayments, or coinsurance required by your insurance plan.

Yes. You can securely pay any outstanding balances through our patient portal or by calling our billing department directly.

18. Privacy & HIPAA

Absolutely. We utilize state-of-the-art, HIPAA-compliant electronic medical record systems and encrypted telehealth platforms. Your privacy and the security of your Protected Health Information (PHI) are our highest priorities.

No. Virtual telehealth visits are never recorded. They are securely encrypted live video and audio streams, exactly as private as a conversation in a closed exam room.

19. Emergencies

If you are experiencing a life-threatening emergency—such as severe chest pain, difficulty breathing, sudden weakness, or uncontrolled bleeding—you must call 911 or go to the nearest emergency room immediately. Primary Clinic Senior Care is a primary care practice and is not equipped to handle immediate, life-threatening emergencies.

For non-life-threatening urgent issues that occur outside of our regular business hours, our answering service will direct your call to an on-call provider who can assist you.
