Patients receiving services from Primary Clinic Senior Care consent to evaluation, diagnosis, treatment, care management, preventive services, chronic disease management, medication management, and coordination of healthcare services.

Services may include:

• Primary care services
• Preventive care
• Chronic disease management
• Medication management
• Care coordination
• Home health coordination
• Advanced Primary Care Management (APCM)
• Remote Patient Monitoring (RPM)
• Telehealth services
• Laboratory ordering and review
• Diagnostic testing coordination
• Specialist referrals
• Review of medical records

Patients are responsible for providing accurate medical information and following treatment recommendations.

Primary Clinic Senior Care does not provide emergency medical services. Patients experiencing a medical emergency should call 911 or seek immediate emergency care.
