Medicare Assignment & Financial Responsibility

Primary Clinic Senior Care accepts Traditional Medicare and may participate with other insurance programs.

Patients authorize Medicare and other insurance carriers to pay benefits directly to Georgia Primary Care LLC dba Primary Clinic Senior Care for covered services.

Patients remain responsible for any applicable:

• Deductibles
• Coinsurance
• Copayments
• Non-covered services

Medicare-covered care management programs may include patient cost-sharing obligations depending on insurance coverage.

Patients should notify the practice of insurance changes.
