Primary Clinic Senior Care is committed to protecting the privacy and confidentiality of your protected health information ("PHI"). Appointment Reminders and Care Communications

We may contact you by telephone, voicemail, text message, email, mail, patient portal, or other approved communication methods regarding appointments, laboratory results, medication information, care coordination activities, referrals, home health coordination, APCM services, RPM services, billing matters, and other healthcare-related communications.
