Annual Wellness Visits

Coordinated Medicare primary care designed to help seniors remain healthy, independent, and safely at home.

Building Your Blueprint for Health

As we age, healthcare should focus on prevention rather than simply reacting to illness. The Medicare Annual Wellness Visit (AWV) is a unique, dedicated appointment designed entirely around proactive care planning. It is not a traditional physical exam; it is a comprehensive conversation about your current health status and your future risks.

At Primary Clinic Senior Care, we utilize the Annual Wellness Visit to establish a deep, holistic understanding of your life. We assess your home safety, evaluate your cognitive health, discuss advance care planning, and map out the preventive screenings you need for the upcoming year. This allows us to deliver high-touch, concierge-level coordination customized to your exact lifestyle and goals.

Real-World Outcomes

Preventive Focus

By thoroughly reviewing your risk factors, we can schedule necessary cancer screenings, bone density scans, and immunizations to keep you healthy.

Fall Prevention

We rigorously assess your fall risk and home environment, ordering physical therapy or assistive devices before a debilitating fracture occurs.

Cognitive Baseline

Annual cognitive screenings allow us to detect subtle memory changes early, ensuring that you and your family have time to access specialized support.

Who should schedule an AWV?

Every Medicare Part B beneficiary who has been enrolled for more than 12 months is eligible for an Annual Wellness Visit. It is especially beneficial for those dealing with:

  • New concerns regarding memory or confusion
  • Multiple complex medications prescribed by different doctors
  • A recent history of falls or unsteadiness at home
  • The need to discuss and document end-of-life wishes (Advance Directives)

Zero Out-of-Pocket Cost

Medicare highly encourages preventive care. Therefore, the Annual Wellness Visit is covered at 100% by Medicare Part B, with no deductibles or copays when you see a participating provider.

What to Expect

1

Health Risk Assessment

Before the visit, you will complete a comprehensive questionnaire about your medical history, daily routines, and safety at home.

2

Virtual Consultation

You will meet with your provider via our secure telehealth platform. We will review your medications, screen for cognitive impairment, and assess your fall risk.

3

Your Personalized Care Plan

Following the visit, you will receive a 5-to-10-year preventive care screening schedule, along with referrals to specialists or home health agencies if needed.

Frequently Asked Questions

Is the AWV a physical exam?

No. The AWV focuses strictly on preventive care planning. While we will check routine metrics like blood pressure and weight, it does not involve the hands-on diagnostic exams seen in a traditional physical.

What if I have new symptoms to discuss?

If you need to discuss a new illness, pain, or medical issue during the visit, we can transition the appointment into a "sick visit." Be aware that standard copays or deductibles apply to the sick visit portion.

Can a family member attend the visit with me?

Absolutely. We strongly encourage family members or caregivers to join the virtual visit to help answer questions and understand the personalized care plan moving forward.
