Remote Patient Monitoring

Coordinated Medicare primary care designed to help seniors remain healthy, independent, and safely at home.

Continuous Care Between Visits

For seniors managing complex, chronic conditions, checking vitals only once every few months at a doctor's office is simply not enough. Blood pressure spikes and irregular heart rhythms often occur unexpectedly, and if left unnoticed, they frequently lead to emergency room visits.

Our Remote Patient Monitoring (RPM) program provides you with cellular-connected medical devices that automatically transmit your vital signs directly to our clinical team. This continuous stream of health data allows us to act proactively. We no longer wait for you to feel sick; instead, we monitor trends, detect early warning signs, and intervene with medication adjustments days before a crisis occurs.

Real-World Outcomes

Fewer Hospitalizations

By detecting subtle changes in weight or blood pressure early, we can prevent the severe exacerbations that typically force seniors into the hospital.

Medication Safety

We can immediately assess whether a newly prescribed blood pressure or heart medication is working effectively or causing adverse reactions.

Peace of Mind

Patients and their caregivers experience profound relief knowing that a dedicated medical team is actively watching over their vital signs every single day.

Who benefits from Monitoring?

Medicare strongly supports the use of RPM to help seniors manage persistent illnesses. Our clinical team utilizes this technology primarily to manage and stabilize:

  • Hypertension (High Blood Pressure)
  • Congestive Heart Failure (CHF)
  • Diabetes Mellitus
  • Chronic Obstructive Pulmonary Disease (COPD)
  • Weight management related to fluid retention

Effortless Technology

Our monitoring devices are designed exclusively for seniors. There is no Bluetooth pairing, no Wi-Fi setup, and no complicated smartphone apps required. You simply take your reading, and the device automatically securely transmits the data to our clinic.

What to Expect

1

Device Delivery

Eligible patients will receive a designated device—such as a cellular blood pressure cuff or digital scale—shipped directly to their home.

2

Daily Readings

Patients take their readings according to their care plan. Our software securely collects this data and alerts our clinical staff if numbers fall out of a safe range.

3

Proactive Outreach

If we notice a concerning trend, our team will call you to discuss your symptoms, review your diet, or adjust your medications immediately to resolve the issue.

Frequently Asked Questions

Is Remote Patient Monitoring covered by Medicare?

Yes. Medicare recognizes RPM as an essential part of chronic disease management. Standard Medicare Part B coinsurance and deductibles generally apply.

Do I need a smartphone to participate?

No. Our devices have built-in cellular connections, functioning much like an independent cell phone. They send data directly to our clinic without requiring a smartphone, app, or home internet connection.

What happens if my reading is dangerously high?

If a reading triggers a critical alert in our system, our clinical team is notified and will reach out to evaluate your condition. Please note, RPM does not replace emergency services; if you experience a severe medical emergency, always call 911.
